Thank you for considering Physical Therapy for your patients!  Physical Therapy is a conservative, cost-effective option that works!
 
We believe that a good working relationship between therapist and physician is in the patient's best interest for a good outcome. 
 
Copies of your clinical notes and any diagnostic testing results are always appreciated.  The patient can bring those with her to her first appointment, or they can be faxed to us.
 
You will recieve regular reports on your patient's status as well including an evaluation summary.  We are also always happy to discuss the plan of care and patient progress.  Call anytime!
 
 
How to Refer a Patient
Call or e-mail us with the patient's name, phone number and diagnosis.  Or have the patient call so that we can find an appointment time that works best for her!  Referral need only state "evaluate and treat", the patient's name, date and diagnosis with your signature.

Who Should Be Referred?
Patients who should benefit from physical therapy and program options:
 
Pelvic Pain:
Patients with chronic vaginal or pelvic pain, pain with intercourse, pain with tampon use or pelvic exam, pelvic floor tension, coccydynia, vagissimus, vulvodynia, interstial cystitis
 
Pregnancy and Post-partum:
Patients who have back, sacral, hip, pelvic, rib pain, sciatica, carpal tunnel, thoracic outlet or other nerve symptoms  (pain, tingling, numbness),  decreased ability to do normal daily activities, need for strengthening (difficulty lifting, pushing, pulling, or with any normal activities), need for increased flexibility (tight, painful muscles, difficulty moving), desire to start or continue an aerobic exercise program, incontinence
 
Incontinence:
Patients with urinary urgency and/or frequency, stress incontinence, mixed incontinence, incontinence duiring or following pregnancy, incontinence after abdominal or pelvic/vaginal surgery, pain with incontinence (in the low back, hip or abdomen with other medical diagnoses ruled out), inability to contract the pelvic floor, prolapse
 
Post-surgical:
Patients who have had mastectomy, hysterectomy, other abdominal or vaginal surgery, who have pain or tenderness that is more intense, or lasts longer than expected, who have a desire to begin an exercise program and need guidance and/or monitoring, who have difficulty returning to a previous exercise program due to post-surgery condition, difficulty returning to normal daily activities, decreased range of motion or sensation of 'pulling' or tightness during range of motion, decreased strength or flexibility, abnormal scarring or adhesions.
